Birth Registration. It is mandatory to register every birth that occurred in the national territory, the only exception is not yet integrated Indians. In the district of João Pessoa, the 1st. Civil Registry for national citizens is CARTÓRIO AZEVÊDO BASTOS, which is in Avenida Presidente Epitácio Pessoa, 1145, close to the Post Office in front of the Banco Rural.
BIRTH The parents can register the birth at the Maternities, provided they have a convention with the Cartorio Azevedo Bastos, from Monday to Friday (except holidays), at the defined hours, seeking the Official Registry scrivener or any other authorized scrivener who will fill out a birth certificate, to be checked/reviewed for data exactness and signed by the declaring person. The certificate will be handed out immediately. The registration done at the maternities with a “Convention " is optional, as a convenience to the declarant, regulated by the Provision Act 05/2004 of the “Corregedoria Geral da Justiça do Estado da ParaÃbaâ€. For the parents that had their children born at the maternities above but the registration is not “convenciodao†or which have not declared the birth within the first days of birth and they reside in this district, the registration can be made at the Cartório Azevêdo Bastos gratuitously. The attendance is made immediately and the birth registration certificate is made within the hour, with the issuing of the traditional birth certificate printed on official Notary Public paper money like stationary in total assurance and quality, without any costs to the parents. The working hours are Monday to Friday from 08:00 to 12:00 and from 13:30 to the 17:30. The parents can, if they desire, go to the Registro Civil (Civil Registry) of the district of their residence or to the Registro Civil of the district of the hospital to register the child’s’ birth. As to the birth registration, the following guidelines should be observed: 1.   Presentation, the yellow paper which is the declaration that the baby was born alive which is, at time of delivery, supplied by the Hospital; 2.   If the parents are married in Brazil, the father's or mother's presence is enough, providing ID card (or equivalent document) and marriage license; if married abroad, the certificate should be transcribed to the Book " E " which is a registration service done by the 1st. Registro Civil da Comarca de João Pessoa, providing one of the parents is Brazilian) or at the Registro de TÃtulos e Documentos, if both are foreigners of the home District, after consular legalization and documents translated by an official sworn-in translator. 3.   If the parents are single, the two should attend, providing their ID card or equivalent document; if the mother is younger than sixteen years, she should be accompanied by her father or mother; it is also possible, for just one of the genitors attend, if he/she presents a declaration of consent by the parents with authenticated signatures), to validate this birth certificate. 4.  There is no need of the presence of any witness according to the Federal Law 9.997/2000; 5.   The child should be registered within 15 (fifteen) days after the birth. In case the mother attends the registry, she has 60 (sixty) days after the birth to do so; Prior to registration, it is recommended for the parents to choose the child's full name.
