Transcription of Death.

   

   Necessary documents for the Transcript: 

     Foreign Death Certificate 

  1.   Original death certificate from the country of origin (where the death occurred) legalized by the Brazilian Consulate or Embassy. 
  2. Translation of the certificate done by a public sworn-in translator in Brazil
and registered at the registry of Titles and Documents (Cartório de Títulos e Documentos)
  3. Birth certificate and, if the case, marriage license of the departed according to article 106 of the Law 6015/73, Only ORIGINAL OR AUTHENTICATED COPY. 
  4. Declaration under the terms of Article 80 of the Law 6015/73, signed by the closest adult relative of the departed with authenticated signature. 
  5. The doctor's certificate attesting the death and containing the cause of death               duly legalized by the Brazilian Consular authority and translated by a sworn-in public translator in Brazil and registered at Registry of Titles and Documents (Cartório de Títulos e Documentos)
  6. Request to make the transcription under the terms of article 32 of the Law 6015/73 with authenticated signature.  
  7. The petitioner for the transcription should bring his/her original ID.

Comment:      The document, as mentioned in item 5, should be presented if the required information is not included in the presented certificate. 

 

    Death Certificate from the Brazilian Consulate or Embassy 
  1. Original death certificate sent by the Consulate or Embassy of Brazil; 
  2. Include the necessary documents as mentioned in items 3, 4, 5 and 6 of this information. The document of item 5 should be presented if it doesn't have this information in the presented certificate.  
  3. The petitioner for the transcription should bring his/her original ID. 

      Attention: In the absence of the closest relative (item 4) he/she can be re-presented by a recent “per procura” Power of Attorney.

   Observation: If the re-presentation “Procura” is personal, it should be with authentication of the signatures according to (Art 654#2nd of the Civil Code). 

      The cost for registration is applied according to the official cost schedule.
